New Patient Registration

You will be asked to complete a registration form. This can be downloaded from this website. A ‘new patient check’ with a practice nurse may be offered. An appointment with a doctor is necessary if you are on regular medication.

As a group practice you may see whichever doctor you choose. You are not necessarily obliged to see the doctor with whom you are registered. Urgent medical problems will be referred to the Duty Doctor.

At times it might not always be possible to see the doctor of your choice and if that were to be the case, an explanation will be offered.

The practice area covers Devizes and outlying villages up to a radius of 7 miles from Devizes. These include, among others, All Cannings, Bishops Cannings, Bromham, Potterne, Poulshot, Rowde, Seend, and Worton.
