The Practice
Premises
All consulting/treatment rooms are on the ground floor. A wheelchair is available if required and loop system is fitted at reception for the hard of hearing. There is a toilet for the disabled and there are baby changing facilities. We are happy to provide a quiet area should a mother need to breast feed. There is limited parking space only for use when visiting the surgery and one disabled parking space available.
Registration For New Patients Who Live In Or Near Devizes
If you live in or near Devizes and wish to register with this practice, you will need to complete this Registration Form and New Patient Questionnaire. You may be offered a ‘new patient check’ with a nurse and, if on medication, an appointment with a doctor. Fees
Fees are payable for Non-NHS work such as Insurance Claims, medical examinations and passport signing. A full list of items for which there is a charge is available from reception.
